CVHN and our members are united in our goal to improve population health, provide high quality patient experiences in the delivery of care, and to effectively decrease the cost of care for the communities we serve. The goal of a patient centered health home is to provide timely comprehensive quality primary care to patients and families that is based on the family’s values, needs, and concerns.
CVHN provides member health centers with learning sessions, webinars, resources, and tools that support clinical, operational, and financial quality improvement projects through data driven decision-making.
CVHN partners with our state association, California Primary Care Association (CPCA), to provide a National Committee for Quality Assurance (NCQA) Content Expert and Coach to assist member centers engaged in the transformation process to become recognized patient-centered health homes.
