What We Do

Central Valley Health Network (CVHN) works alongside health center members to support the delivery of high-quality, accessible care across the Central Valley, Sacramento, and Northern California regions. Through Quality Improvement and Population Health Management efforts, CVHN strengthens member capacity by offering practical tools, technical support, and opportunities for peer learning to improve care delivery and patient outcomes.

CVHN also provides program service support to FQHC members in important areas to improve the delivery of primary care services to uninsured and underserved communities:

Workforce Development

The preparation, training, recruitment, and retention of highly qualified health care professionals is a primary focus of CVHN’ s workforce development program. The program supports our community health center members in their work to serve the needs of the diverse and medically underserved population throughout the regions we serve in the Central, Northern and Sacramento regions of California.

QI & Population Health Management

Central Valley Health Network (CVHN) supports fellow health center members in delivering high-quality, accessible care across the Central Valley and Northern Sacramento region. Through a comprehensive suite of Quality Improvement and Population Health Management efforts, we equip our membership with practical tools and peer learning opportunities to strengthen care coordination, address health equity gaps, and improve patient outcomes.

Central Valley AHEC (CVAHEC) addresses the shortage and maldistribution of healthcare professionals by:

  • Partnering with academic institutions to connect students to meaningful, community-based clinical training experiences for health professions, students, and residents
  • Providing educational opportunities for the current and future primary care workforce
  • Recruiting students from minority and disadvantaged backgrounds into health careers

The CVAHEC is part of the national and state AHEC system and serves 20 counties in the San Joaquin Valley, Sacramento Region, and Northern California.

CVAHEC offers the following services and programs:

  • AHEC Scholars
  • Education for Healthcare Professionals

Quality Improvement Collaborative

In 2021, CVHN launched a Quality Improvement (QI) Collaborative in partnership with the Weitzman Institute, offering a QI peer network serving 20+ health centers in the Central Valley. The effort, funded by a Health Resources Service Administration (HRSA) grant through the California Primary Care Association, includes three objectives:

  • UDS-defined clinical quality measures
  • Social determinants of health (SDOH)
  • Emergency Preparedness

We partner with the Weitzman Institute to focus on the first two objectives, clinical quality measures and social determinants of health

On a monthly basis, sessions are held to provide training and technical assistance, sharing of best practices, and data analysis of clinical quality measure improvement. Over the past two years, this collaboration has focused on improving performance measures selected by CVHN and participating member health centers.

An Advisory Group consisting of six health center leaders is convened monthly by CVHN with WI support and served to guide and provide feedback on the collaborative experience.

Clinical Quality Collaborative Online

Individuals who participate in the QI Collaborative have secured access to our CVHN Clinical Quality Collaborative group on the CPCA Connected Community platform. Within this peer network space, you may share best practices via discussion threads, view previous session materials, and access resources.
