At CVHN, our quality improvement work begins with strong partnerships that drive meaningful, measurable change across the region. As a Regional Consortium of Community Health Centers, we work alongside our health centers to align improvement priorities and advance strategies that close equity gaps and enhance patient experience. Our approach is grounded in a shared commitment to population health, continuous learning, and scalable solutions that meet the unique needs of diverse patient communities.
Quality Improvement & Population Health Management
Building improvement capacity across our member health centers is a core focus of our work. Via hands-on support, curated resources, and structured opportunities for peer learning, we assist teams to strengthen their quality improvement efforts and implement evidence-based strategies. CVHN supports members in advancing shared strategic quality improvement priorities, navigating challenges, and spreading effective clinical and operational practices across the regions served.
Collaborative Focus Areas
Central Valley Health Network (CVHN) offers a Quality Improvement Collaborative that serves as a peer network for 20+ health centers across the Central Valley, Sacramento, and Northern California regions.
Through this partnership, CVHN provides training and technical assistance, facilitate the sharing of best practices, and support data analysis to drive performance measure improvement. Over time, the collaborative has focused on strengthening performance measures while creating a dedicated space for cross-center learning and engagement.
Each year, the QI Collaborative’s focus areas evolve to address key preventive care and chronic disease management measures.

Quality Improvement Advisory Group:
CVHN partners with members to advance our Quality Improvement (QI) and Population Health Management initiatives through the QI Advisory Group. This dedicated subset of leaders from health centers across the Northern Sacramento and Central Valley regions provides strategic input on Collaborative priorities and program design. Their guidance helps ensure that topics, resources, and technical assistance remain responsive to the evolving needs of member organizations and the patient communities they serve.

CalAIM is a statewide initiative led by the California Department of Health Care Services (DHCS) that aims to transform Medi-Cal by advancing whole-person care, improving health equity, and addressing social drivers of health. Through key components like Enhanced Care Management (ECM) and Community Supports, CalAIM helps connect patients to services that go beyond clinical care and address health-related social needs.
Central Valley Health Network (CVHN) serves as a regional convener and facilitator, assisting member health centers to stay informed, connected, and prepared as CalAIM continues to evolve. We support this work by hosting shared learning forums, disseminating policy, implementation, and funding updates, and connecting health centers with external partners and community-based organizations (CBOs) that complement ECM and Community Supports.
A key resource in this effort is CVHN’s Community-Based Organization (CBO) Referral List, a regional directory covering 19 counties across the Central Valley and Northern Sacramento regions. The tool connects health centers with hundreds of local Managed Care Plan–contracted providers and non-contracted organizations offering services aligned with CalAIM’s Community Supports, including housing navigation, food and nutrition services, transportation assistance, behavioral health resources, and more.
